Job summary

East London NHS Foundation Trust is seeking a Crisis Practitioner - Nurse to join the Crisis Resolution Home Treatment Team. The role involves working within a multidisciplinary service to assess referrals, conduct home treatment, and deliver risk-based care to individuals in mental health crisis.

You will contribute to gatekeeping, collaborate with GPs, CMHTs and acute wards, and promote recovery through psycho-social interventions and early discharge planning.

Responsibilities

• Assess referrals and provide home treatment for patients on caseload.
• Be part of the assessment or home treatment team within the Crisis Team.
• Provide gatekeeping and feedback based on CRHTT criteria.
• Provide comprehensive risk assessments including baseline physical assessments.
• Provide expert advice on management, interventions and diagnosis during mental health crises.
• Support acute hospital wards and facilitate early discharge of patients or support them on leave.
• Utilise psycho-social interventions to improve mental health outcomes.
• Reduce readmissions by client education and service-user led recovery and longer-term support.
• Coordinate staff workload as a shift co-ordinator.
• Adhere to lone worker policy and supervise medication administration when required.
