Job summary

Essex Partnership University NHS Foundation Trust is seeking a Mental Health Practitioner to join a fast-paced urgent community care pathway. You will work with the Ambulance Paramedic clinician to provide immediate crisis care in the community across West Essex and support Hertfordshire as needed.

You will conduct initial assessments, formulate care plans, and deliver NICE-guided interventions, with documentation and risk assessment responsibilities across a 24-hour service.

Responsibilities

• Provide triage screening and crisis response in the community
• Conduct initial assessments and develop collaborative care plans
• Supervise junior staff when required
• Oversee documentation and risk assessment of patients
• Deliver education on medications and adherence
