Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Structured Clinical Management Practitioner to work within the Complex Needs Service, delivering SCM and NICE-recommended therapies in a community setting.

The role supports adults with complex emotional difficulties, collaborates with carers, participates in regular supervision, and manages a capped caseload across assessment, stabilization and intervention phases.

Qualifications

• Applications welcomed from a variety of professional backgrounds, including nursing, social work and occupational therapy.
• Flexible working considered where possible.
• Cross-site travelling within East-Cheshire may be required.

Responsibilities

• Support the implementation of the SCM pathway for clients with complex emotional difficulties, including assessment, stabilization and intervention phases.
• Deliver evidence-based interventions in a community setting and maintain a limited, relational caseload.
• Attend and contribute to regular SCM supervision sessions and training opportunities.
• Collaborate with carers and other professionals to promote joint care planning and patient engagement.
