Job summary

Cambridge University Hospitals NHS Foundation Trust is seeking a Band 6 Complex Discharge Specialist Nurse to work within the Complex Discharge Team. You will coordinate discharges from hospital, liaising with ward staff, community teams, patients and families to plan safe and timely pathways.

You will contribute to a MDT approach, support complex discharge planning and help implement the Home First pathway where appropriate while working seven days a week, including weekends and bank holidays.

Qualifications

• Experience in discharge planning and coordinating with multidisciplinary teams.
• Ability to liaise with community partners, patients, families and carers.
• Knowledge of safeguarding and patient safety in discharge processes.

Responsibilities

• Provide seven-day discharge planning service including weekends and bank holidays.
• Assess patients’ needs and agree the most appropriate discharge pathway.
• Coordinate referrals for care and support across discharge pathways.
• Act as MDT member to progress patient journeys.
