Job summary

Active Care Group is seeking a Complex Care Support Worker to provide person-centred home care for clients with complex health needs. You’ll work to keep people living safely, comfortably and independently, within their own homes, with support from clinical and care teams.

You’ll handle a range of tasks, from personal care to medication assistance, and adapt plans to each individual's goals. A full UK driving licence and previous complex care experience are required.

Qualifications

• Full UK Driving Licence is required.
• Experience in complex care is essential.
• Ability to work independently with initiative and compassion.

Responsibilities

• Deliver high-quality personal and daily living support in clients' homes.
• Promote independence, dignity, and wellbeing according to care plans.
• Follow personalised care plans and communicate with Care Managers.
