Job summary

AMS Case Management is seeking two support workers to assist a 22-year-old client with cerebral palsy, autism and visual impairment at home and in the community. You will work under therapy professionals, follow a care plan, and help with meals, mobility, and daily routines.

Full induction and DBS checks will be completed. A full clean driving license and two references are required to join a small, collaborative team dedicated to improving the client’s wellbeing and independence.

Qualifications

• Two references required.
• A full enhanced DBS check will be completed after accepting the position.

Responsibilities

• Provide assistance with all of the client’s care requirements.
• Administer prescribed medication and maintain records.
• Support the client to prepare and cook meals.
• Engage the client in activities of his choice, both at home and in the community.
• Promote the client’s wellbeing and happiness.
• Assist with rehabilitation under supervision of doctors and therapists.
• Maintain a safe home and community environment after risk assessment training.
• Follow the care programme agreed with the case manager, therapists, and the client.
• Handle domestic chores (laundry, ironing, cleaning) to ensure a safe environment.
• Raise concerns about the client’s health to the family or case manager.
• Communicate effectively within a small team and attend meetings/trainings.
