Job summary

Qualifications

• Monitoring health changes and reporting to supervisors.

Responsibilities

• Support individuals with personal care, daily living activities and mobility.
• Administer or support medication in line with care plans.
• Promote independence, dignity, choice and wellbeing.
• Monitor and report changes in health.
• Maintain accurate care records and follow plans.
• Safeguard individuals from abuse, neglect and harm.
• Work closely with families, nurses, social workers, and other healthcare professionals.
