Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust is seeking a Technical Care Instructor (Band 3) to join the Home First initiative. The role focuses on delivering safe, person-centred care in patients’ homes and supporting a multi-disciplinary team within the Integrated Urgent Care framework.

Shifts run on a seven-day rota, with early and late patterns, including weekends and bank holidays. Travel is required; access to a vehicle is essential to perform duties effectively.

Qualifications

• Experience delivering care in community/home settings.
• Ability to work within a multi-disciplinary team.
• Strong documentation and compliance with standards.

Responsibilities

• Provide safe care to patients in their homes within the community.
• Coordinate individualised patient care with the Integrated Urgent Care Team.
• Maintain documentation standards and support audits.
