Required:

Main Duties and Responsibilities

• Coordinate with Social Workers, District Nurses, and other agency professionals within the multidisciplinary team.
• Communicate changes in requirements effectively.
• Adhere to company policies and procedures.
• Attend training sessions for personal development.
• Assist customers with personal needs such as dressing, washing, meal preparation, and community access as per the Care Plan.
• Perform light housework in customers' homes, including dusting, vacuuming, cleaning bathrooms and kitchens, polishing furniture, washing and ironing clothes, and meal preparation.
• Encourage customers to undertake personal care tasks they are capable of, recognizing daily variability in abilities.
