A leading organization focused on reducing homelessness is seeking a full-time case manager to support clients in achieving independent living. This role involves assessing client needs, managing caseloads, and collaborating with various agencies to provide comprehensive support. Ideal candidates will possess strong communication skills and a commitment to making a difference in the community.
This is a Permanent , Full Time vacancy that will close in a month at 23:59 BST .
The Vacancy
FULL TIME - 37hrs
HCC social inclusion lot 3 - Hants East - provides supported accommodation for adults who are homeless or at risk of homelessness. This service also provides community / in-reach support to individuals who are placed in temporary accommodation by East Hampshire Council or are having difficulties maintaining their tenancies in their own accommodation.
Our aim is to provide person-centred support to our clients, empowering them to maintain their existing tenancies and / or gain the life skills they require to move on to more independent living.

JOB PURPOSE
To lead on the case management of clients and be part of a team that provides support to clients living in the community to keep them safe and help them achieve their goals.
To provide people with homes and specialist support so they feel more valued and secure, and ready to take the next steps
