Job summary

The Community Support Worker will work as part of the Early Intervention Team providing socially inclusive mental health services to individuals experiencing first episode psychosis. To support Early Intervention Team Practitioners with a specific remit of engaging individuals working towards personal goals and improving and maintaining good physical and mental health.

Main duties of the job

Main duties of the job

  • To highlight any physical health concerns to the Care Coordinator, GP and Consultant Psychiatrist.
  • To refer clients for appropriate interventions and offer health advice
  • To assist Early Intervention Team Practitioners in planning and evaluating individual care plans.
  • To support service users in engaging positively and actively with appropriate services and resources such as benefits, employment, housing and substance misuse services.
  • To promote independent living of service users under the Early Intervention Team through practical help, advice and emotional support.
  • Provide information and support with health promotion
  • Support the Early Intervention Team Practitioners in monitoring the service users’ mental state, level of functioning and progress and escalating any concerns.
  • To report and alert the Early Intervention Team Practitioners as appropriate on progress and change such as the identification of early signs of relapse of illness.
  • Maintain accurate documentation in accordance with Pennine Care NHS Foundation Trust policies and procedures including use of electronic record keeping.
  • Provide information and support to enable service users to engage in meeting education, employment and vocational needs. To include arranging initial contact and attending with the service user if appropriate.
  • Provide information and support to family, friends and carers of service users.

What happens if I am offered the position after interview?

The hiring manager will make contact with you to verbally offer you the position. The hiring manager will then inform Recruitment of the decision and provide relevant paperwork. You will be sent a formal conditional offer via email.

  • Disclosure and barring service (DBS)/Criminal record check (dependent on role)

  • Occupational health assessment
  • Employment history and reference validation

All applicants external to NHS will be required to provide HMRC employment history to cover the most recent three years. This information will used to validate employment history and references as part of pre-employment checks.
