Job Summary

The Community Staff Nurse in the School Nurse Team is a registered general nurse working as part of the public health nursing team, providing preventative and supportive healthcare to children, young people, and their families.

The post holder works with a public health focus under the supervision of a Specialist Community Public Health Nurse (SCPHN) with a defined population.

As part of the public health nursing team, the post holder collaborates with a wider multi-disciplinary and inter-agency team to offer public health services to the community.

The work takes place in various settings, including home, clinic, and community venues.

Base

Our Healthy Family Teams support our Surrey-wide service. You will be based at one of our Office Hubs across Surrey with opportunities for hybrid working.

Main Duties

1. Deliver the healthy child programme at four levels as delegated by the SCPHN.
2. Recognise health needs for individual clients and situations, sometimes in complex scenarios, and refer to Public Health Specialists when necessary.
3. Undertake health assessments in schools.
4. Participate in case conferences and inter-agency meetings, undertaking health needs assessments and care planning.
5. Support SCPHN in providing targeted support for children at risk or with complex needs and ensure Partnership Plus plans are in place.
6. Identify, prioritise, and implement public health programmes responding to community needs.
7. Conduct full assessments of individual public health needs and facilitate support or onward referrals, focusing on sexual health, smoking, and weight management.
8. Offer advice and support to children, young people, and families based on evidence-based practices.
