Job summary

Cardiff and Vale University Health Board is seeking a flexible district nurse to deliver high-quality care in patients' homes and residential settings. You will work with community nurses and multidisciplinary teams to ensure timely assessment and care for residents of Stockport and surrounding areas.

The role requires current NMC registration, GCSE English, and a nursing degree or equivalent. Travel across community sites is essential, with support for DBS clearance as part of the application

Qualifications

• Must hold current NMC registration as a district nurse.
• GCSE English at grade C or equivalent is required.
• Diploma in Nursing Practice or a degree in nursing is expected.

Responsibilities

• Provide district nursing care in patients' homes and residential settings.
• Work with Community Nurses and multidisciplinary teams to deliver care seven days a week.
• Cover the service between 08:00–18:00 and travel across multiple community sites.

Job description

Our work in Core Neighbourhoods is at the very heart of Stockport Neighbourhood Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers., The post holders will work with the Community Nurses and other multidisciplinary teams with the support of Mastercall GPs, to provide a first class service to the residents of Stockport.

We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers. The post holder should have good organisational and assessment skills be flexible and willing to cover the service between 08:00 - 18:00 over a 7 day period. Due to the nature of the role, it is essential that applicants are able to travel independently and efficiently across multiple community sites to meet the operational requirements of the role,

• Developing areas of expertise in community related practice. Tissue Viability/ Frailty/ end of life care
• Experience of audit involvement in project work., Demonstrate knowledge of current local and national health agenda and developments.
• Awareness of current clinical issues in out of hospital care, Demonstrate an understanding of an implication of an issue which could impact on community service provision., Current 1st level registration with NMC
• Grade C / 4 (or equivalent) in GCSE English, Diploma in Nursing Practice or degree
