Job summary

The Confederation, Hillingdon CIC in London is seeking a Social Prescribing Link Worker to join the Primary Care Network multi-disciplinary team. You will assess clients, take referrals from GP practices and partner agencies, and develop personalised care and support plans.

You will connect people to community groups and statutory services, manage a caseload with ongoing support, and collaborate with local partners to address wider determinants of health and reduce health inequalities.

Qualifications

• Educated to NVQ Level 3, Advanced level or equivalent qualifications or equivalent work experience.
• Experience of supporting people, their families and carers in a related role.
• Able to recognise when it is necessary to refer patients to other healthcare professionals/organisations.
• Understanding of equality, diversity and inclusion.
• Knowledge of IT systems, including ability to use word processing, email and internet to create simple plans and reports.
• Knowledge of the personalised care approach.

Responsibilities

• Take referrals from the PCN Core Network Practices and from partner agencies.
• Provide personalised support to individuals and their families to improve health and wellbeing.
• Develop a simple personalised care and support plan and connect people to community groups and statutory services.
• Manage and prioritise your own caseload, with appropriate referrals when needs exceed scope.
• Collaborate with GP practices and MDT meetings to support social prescribing.
