Job summary

South Norfolk and Broadland Councils are seeking a Community Connector / Social Prescriber to help residents access support and improve wellbeing. You will work with individuals and families, receive referrals, and assess needs to connect people with local services.

This role involves collaborating with partners, maintaining records, and delivering social prescribing sessions in GP practices, with flexible working and the chance to make a real difference in the community.

Qualifications

• Experience of working with individuals in distress or challenging circumstances.
• Working knowledge of common health conditions and the benefits system.

Responsibilities

• Understand what matters most to residents and identify opportunities to improve wellbeing.
• Receive referrals and undertake person-centred assessments to identify needs and options.
• Connect residents to local services, community resources and specialist support.
• Develop and maintain strong relationships with local statutory and VCSE partners to improve outcomes.
• Maintain accurate records of interactions and interventions.
• Deliver a small number of social prescribing sessions within GP practices.
