Job summary

South Norfolk and Broadland Councils are seeking a compassionate Community Connector / Social Prescriber to join our Help Hub. You will work directly with residents to understand what matters to them, identify wellbeing opportunities, and connect people to local services and partner organisations.

You will build strong relationships with statutory and VCSE partners, maintain records, and deliver social prescribing sessions in GP practices while supporting residents to improve their health and

Qualifications

• Experience with individuals facing distress or challenging circumstances.
• Working knowledge of common health conditions, substance misuse issues and benefits system.
• Understanding of social disadvantage impact on health and wellbeing.
• Strong written and verbal communication; ability to build relationships.

Responsibilities

• Undertake person-centred assessments to identify needs and support options.
• Refer to local services and community resources; connect people to support.
• Develop and maintain relationships with partners to improve outcomes.
• Deliver a small number of social prescribing sessions in GP practices.
• Maintain accurate records of interactions and interventions.
