Job summary

HCRG Care Group in Swindon is seeking a Specialist Respiratory Practitioner to join a multidisciplinary community respiratory team. You will deliver specialist assessment, monitoring, and treatment for patients referred to pulmonary rehabilitation, COPD supported discharge, and home oxygen assessment services.

Qualifications

• Demonstrated ability to deliver specialist respiratory care.
• Experience working within a multidisciplinary community team.
• Ability to contribute to clinical leadership and service development.

Responsibilities

• Assess, monitor and treat patients referred to pulmonary rehabilitation and COPD pathways.
• Support COPD discharge, home oxygen assessment and complex case management.
• Provide autonomous practice within evidence-based protocols and guidelines.
• Participate in service redesign and quality improvement initiatives.
