Job summary

HCRG Care Group is seeking a Band 7 Respiratory Clinical Team Manager to lead the Community Specialist Respiratory Team in Bath and North East Somerset, Swindon and Wiltshire. You will provide specialist assessment, treatment and management of respiratory patients in community settings and oversee care plans and oxygen prescribing.

You will support workforce development, quality improvement and patient safety while delivering COPD admission avoidance and pulmonary rehabilitation.

Qualifications

• Registration with NMC or relevant Allied Health Professional body.
• Evidence of CPD relevant to respiratory care.
• Significant experience in respiratory medicine/services.
• Advanced knowledge of respiratory assessment, treatment and management.
• Experience supporting patients with long-term conditions and complex health needs.
• Ability to develop and implement individualized care plans.
• Strong clinical reasoning and decision-making skills.
• Experience mentoring or supervising junior staff.
• Ability to work autonomously and manage competing priorities.
• Experience collaborating across organisational and professional boundaries.

Responsibilities

• Provide specialist assessment, treatment and management of patients with respiratory conditions in community settings.
• Develop, implement and evaluate evidence-based patient management plans.
• Support timely intervention to prevent unnecessary hospital admissions and facilitate safe care at home.
• Deliver community oxygen assessments, interpretation of results and oxygen prescribing per guidance.
• Participate in delivery and development of Pulmonary Rehabilitation services.
• Provide clinical supervision, mentorship and support to junior staff and learners.
• Contribute to clinical governance, audit, quality improvement and performance monitoring.
• Work with multidisciplinary and multi-agency partners to ensure safe patient care.
