Job summary

HCRG Care Group in Swindon is seeking a Specialist Respiratory Practitioner to join a multidisciplinary community respiratory team. You will deliver specialist assessment, monitoring, and treatment for patients referred to pulmonary rehabilitation, COPD supported discharge, and home oxygen assessment services.

Responsibilities

Delivering Specialist Respiratory Care in the Community This role provides an opportunity to contribute to the delivery of high-quality, evidence-based respiratory services that improve patient outcomes, support self-management, and reduce unnecessary hospital admissions. The post holder will work as part of an experienced multidisciplinary community respiratory team, providing specialist assessment, monitoring, treatment, and review for patients referred to pulmonary rehabilitation, COPD Supported Discharge, complex case management, early intervention (hospital avoidance), and home oxygen assessment services. Using advanced clinical skills and specialist knowledge, the successful candidate will deliver safe and effective care in accordance with current evidence, best practice, and operational protocols. The role will also contribute to the clinical leadership and development of the community respiratory service, promoting multidisciplinary collaboration and a patient-centred approach to
