Job summary

NHS National Services Scotland seeks an experienced HCSW to join the Community Rehabilitation Team in the Dunfermline Locality. You will work with Occupational Therapy and Physiotherapy to deliver therapeutic programmes in patients' homes or community settings, aiming to prevent hospital admission and aid early discharge.

You will hold an SVQ Level III in Health and Social Care or equivalent, with experience as a support worker desirable; older people experience is preferred.

Qualifications

• SVQ Level III in Health and Social Care or equivalent is required.
• Experience as a support worker is desirable.
• Experience with older people is desirable.

Responsibilities

• Carry out ongoing treatment programmes for a mixed caseload of mainly older people.
• Work with Occupational Therapy and Physiotherapy Services within CRT to deliver therapeutic programmes at service users’ homes or in their community.
