Job summary

NHS Fife is seeking an experienced RSW to join the Community Rehabilitation Team in the Kirkcaldy Locality, delivering treatment programmes in ward, home or community settings. The role collaborates with OT, PT and District Nursing to prevent hospital admission and support early discharge.

Applicants should have SVQ Level III in Health and Social Care or equivalent; experience as a support worker and with older people is desirable.

Qualifications

• SVQ Level III in Health and Social Care or equivalent is required.
• Experience as a support worker is desirable.
• Experience with older people is desirable.

Responsibilities

• Deliver therapeutic treatment programmes to service users in ward, home or community settings.
• Work with Occupational Therapy, Physiotherapy & District Nursing to coordinate care and prevent hospital admission.
• Carry out ongoing treatment programmes for a mixed caseload, primarily older people, in a community setting.
