Job summary

NHS Scotland in Kirkcaldy is seeking an experienced Registered Social Worker (RSW) to join our Community Rehabilitation Team (CRT). You will deliver therapeutic programmes in ward, clinic, or patient homes, coordinating with Occupational Therapy, Physiotherapy and District Nursing to support early discharge and prevent hospital admission.

The role focuses on daily community care across a mixed caseload, predominantly older people, with 24.7 hours across 7 days and regular supervision plus PDP

Qualifications

• SVQ Level III in Health and Social Care or equivalent.
• Experience working as a support worker would be desirable.
• Experience working with older people is desirable.

Responsibilities

• Deliver therapeutic treatment programmes to service users in the ward setting or their own homes.
• Carry out daily care in the community, a service provided 365 days a year from 07:45–22:00.
• Work within a multidisciplinary team with Occupational Therapy, Physiotherapy and District Nursing to prevent hospital admission and support early discharge.
