Job summary

Choice Care in Sheffield is seeking an Outreach Rehabilitation Support Worker to join a dynamic, collaborative team. You will work within the community, in clients' homes, and local settings, supporting individuals to regain skills and improve quality of life after injury.

A driving licence and access to a vehicle are essential, and full training is provided. This role offers varied days, a supportive learning environment, and opportunities to contribute to client-centred care while developing

As this role is in the community supporting individuals in theo own homes, communities and workplaces, it is essential to be a driver and car owner. This role includes working on a varied rota including weekend work (If preferred, there may be the opportunity for you to do some work in our residential rehab service)
