The role offers flexibility as full or part-time, permanent employment, and involves working weekdays with afternoons and sleeping nights.

You will provide high-quality, person-centred support, helping with daily living, therapy, and community access while collaborating with a multidisciplinary team.

Qualifications

• Experience in care, support, health or related role required.
• Experience with acquired brain injury or neurological disabilities beneficial but not essential.
• Willingness to undertake specialist training and professional development.
• Full UK driving licence essential.

Responsibilities

• Provide high-quality, person-centred support under supervision of the Case Manager and multidisciplinary team.
• Support independence, daily living activities, meal prep, personal care, mobility, and community access.
• Assist with therapy programmes and rehabilitation recommendations in daily routines.
• Work with family, therapists and other professionals to maximise outcomes.
• Attend medical, therapy, social and recreational appointments.
• Maintain accurate records and assist effective communication within the team.
• Drive the client’s vehicle to enable access to community activities and appointments.

Overview

We are seeking motivated, dedicated, and enthusiastic Support Workers to join a specialist rehabilitation support team, supporting a young woman living with the long-term effects of an acquired brain injury. Our client lives in her own home and leads a full and active lifestyle. She enjoys spending time in the community, socialising with friends and family, shopping, eating out, attending appointments, and taking part in a range of leisure activities. She is able to direct her own care and enjoys being actively involved in decisions about her daily life, support, and future goals.

As part of her ongoing rehabilitation, she is supported by a multidisciplinary team of therapists and professionals. Support workers play an important role in helping her engage with therapeutic programmes and implement rehabilitation strategies within everyday activities, helping her maximise her independence, confidence, and quality of life. She also has a much-loved dog who is an important part of her daily life, so applicants must be comfortable around dogs and enjoy working in a dog-friendly environment.

As a Support Worker, You Will:

• Provide high-quality support in accordance with best practice, agreed support plans, and under the direction of the Case Manager and wider multidisciplinary team.
• Deliver person-centred support that enables the client to maintain her independence, pursue her interests, and achieve her personal goals.
• Support all aspects of daily living, including personal care, meal preparation, medication administration, mobility, and community access.
• Work collaboratively with family members, therapists, and other professionals to support rehabilitation and maximise outcomes.
• Assist with implementing therapy programmes and rehabilitation recommendations within daily routines and activities.
• Support attendance at medical, therapy, social, and recreational appointments.
• Maintain accurate records and contribute to effective communication within the support team.
• Drive the client's vehicle to enable access to community activities, appointments, and social opportunities.

Experience:

• Previous experience in a care, support, health, rehabilitation, education, or related role is required.
• Experience of supporting individuals with acquired brain injury, neurological conditions, physical disabilities, or complex needs would be beneficial but is not essential, as full specialist training and ongoing support will be provided.
• This role would particularly suit applicants with an interest in rehabilitation, acquired brain injury, psychology, occupational therapy, speech and language therapy, nursing, social care, physiotherapy, or allied health professions.
