East Sussex Healthcare NHS Trust is seeking an Occupational Therapist to join our Joint Community Rehabilitation Service in Lewes. You will work as part of an integrated, multi-professional team delivering person-centred rehabilitation in patients' homes and community settings.

You will undertake holistic assessments, develop care plans, and provide tailored OT interventions to improve independence and quality of life, with regular supervision and opportunities for professional development.

Qualifications

• CPD evidence of continuing professional development.
• Knowledge of client-centred practice and governance.
• Experience working in an NHS or equivalent health system.

Responsabilités

• Undertake holistic assessments and key work on individual cases.
• Develop and implement occupational therapy treatment plans.
• Collaborate with multi-disciplinary teams to support rehabilitation.
