Job summary

East Sussex Healthcare NHS Trust is seeking a part-time Health Care Support Worker to join an integrated Home Care/Joint Care Reablement (JCR) service in Lewes. You will work in clients’ homes, provide rehabilitation, and support earlier discharges from hospital within a seven-day service.

CPD and supervision are embedded; travel across the county is required. DBS check is mandatory; enhanced disclosure applies.

Qualifications

• GCSE grade 4 or GCE C and above.
• Level 2 functional skills or health care support worker apprenticeship / NVQ level 2.
• Level 3 apprenticeship for example senior health care support worker or NVQ level 3 or equivalent knowledge/experience.

Responsibilities

• Key work non-complex clients and provide rehabilitation support in their own homes.
• Respond to urgent referrals and assist clients to stay independent at home.
• Travel around East Sussex to visit clients in their own homes as part of the integrated team.
