Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking an Assistant/Associate Practitioner to support registered practitioners in the Mid Hampshire IIC Therapy Team. The post focuses on community rehabilitation and visiting patients at home across Andover and Winchester, linking closely with GP practices in Mid Hampshire.

You will perform patient assessments under supervision, implement care plans, and help run clinics or groups.

Qualifications

• Perform patient assessments under supervision from a registered practitioner.
• Implement and evaluate care plans under supervision.
• Carry out environmental assessments and arrange equipment as needed.
• Run and facilitate clinics or group sessions.
• A full UK driving licence and access to a vehicle required.

Responsibilities

• Support registered practitioners in the integrated care team within NHS guidelines.
• Maintain excellent communication with patients, relatives and team members.
• Organise, run and facilitate clinics or groups.
