University Hospitals Coventry and Warwickshire NHS Trust is seeking a community nurse to deliver patient-centred care in patients' own homes. You will work with a multidisciplinary team, providing nursing interventions and supporting independence for adults 18+.

Job description

The post will involve working with healthcare assistants and clinicians to provide nursing interventions to patients in their home environment, maximising their independence and providing care to manage long term conditions as well as acute situations. You must be innovative in your approach and willing to work with other members of the integrated team to ensure a high quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training to extend your community nursing skills. Applicants who do not have current valid UK NMC registration will not be considered for this post. Successful candidates must have post-preceptorship experience. Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. You will be able to demonstrate real enthusiasm for community nursing, alongside a commitment to ongoing professional development. The team provides nursing care to housebound patients aged 18 and over within their own homes or chosen place of residence. You will work autonomously, supported by the Community Nursing Sister and Team Lead, delivering a range of nursing interventions and patient-centred care. The service operates 7 days a week from 8am to 8pm, with flexible working available on request to support work-life balance. A car owner/driver with business-use insurance cover is essential for this role.,

• To demonstrate communication and motivational skills to facilitate patients to manage their health. The post holder will need to carry out the assessment, planning and delivery of interventions to a defined case load of patients. There may be complex and difficult issues to overcome to provide safe and effective care e.g. disengagement and refusal of service or high levels of risk to self and others supported by a senior clinician when appropriate.
• To communicate accurate and sensitive information to patients and carers using an appropriate and accessible medium, taking into consideration relevant factors such as background, culture, gender, age and any physical, sensory or learning difficulties. This communication may be complex or sensitive and needs to be modified as required to ensure effective understanding.
• To respect the dignity and preferences of patients and obtain consent to nursing intervention, in some circumstances you would be working in the patients' best interests where consent cannot be gained due to capacity.
• Provide support and education to patients and carers and other relevant individuals considering patient confidentiality.
• Provide support and education to members of the multiprofessional team.
• To liaise with other team members, local services, and agencies to ensure the seamless transfer of patients within the pathway
• The post holder will be required to manage and prioritise their own time to ensure this efficiently meets the needs of the service, and when required to prioritise the work of other staff. This is all within a context of competing demands and fluctuating needs and risks.
• Ensure that discharge planning is commenced as soon after admission onto the nursing case load as is practicable and that patients/carers/family are empowered to make informed decisions affecting their future.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group. If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
