University Hospitals Coventry and Warwickshire NHS Trust is seeking a community nurse to deliver patient-centred care in patients' own homes. You will work with a multidisciplinary team, providing nursing interventions and supporting independence for adults 18+.

The role requires autonomous practice with supervision from a Community Nursing Sister and Team Lead. Applicants must hold UK NMC registration with post-preceptorship experience and be willing to cover a 7-day service window.

Qualifications

• Must hold current UK NMC registration and be post-preceptorship experienced.
• Demonstrated ability to assess, plan and deliver home-based nursing care.
• Commitment to ongoing professional development and patient-centred care.

Responsabilités

• Carry out assessment, planning and delivery of nursing interventions for housebound patients.
• Communicate sensitive information to patients and carers with consideration of culture and needs.
• Respect patient dignity and obtain consent where possible, acting in patients' best interests when needed.
• Provide education and support to patients, carers and multiprofessional team.
• Coordinate with local services to ensure seamless transfers within the pathway.
• Manage own time and prioritise workload to meet service demands.
• Initiate discharge planning promptly after admission onto the nursing caseload.
