Job summary

Hampshire County Council is recruiting a Community Reablement Assistant (Level 2) to support adults over 18 at home and maximise independence. You will conduct initial home visits and perform reablement interventions including personal care, continence care, and moving and handling.

Based in North Hampshire, you may travel to towns such as Basingstoke, Alton, Aldershot, Liphook, Whitchurch, Bordon and Farnborough.

Qualifications

• Knowledge of reablement programmes and person-centred care.
• Experience in social or health care settings.
• Willingness to work flexible hours including evenings/weekends/public holidays.

Responsibilities

• Provide person-centred care during initial home visits and reablement interventions.
• Deliver outcomes through risk assessments, goal setting and plan updates.
• Coach and mentor Assistants and support multidisciplinary teams; travel within Hampshire.
