Job summary

The Bank Community Advanced Assessment Nurse role at CNWL NHS Foundation Trust involves delivering rapid assessment and treatment for urgent community care needs, in partnership with acute, social services and primary care colleagues to safely avoid unnecessary hospitalisation.

You will lead on complex clinical assessment, make autonomous decisions, provide supervision to Band 6 and junior staff, and support the Hospital at Home/UCR pathways while ensuring high-quality, person-centred care

• Lead on advanced clinical assessment and management of patients with complex and acute needs.
• Provide leadership, supervision and mentorship to Band 6 and junior colleagues.
• Coordinate rapid response and community-based care to avoid unnecessary hospital admissions.
