Caseload Management — Act as Care Coordinator under the Care Programme Approach (CPA), leading care planning and monitoring recovery outcomes.
Clinical Assessment & Intervention — Conduct comprehensive biopsychosocial assessments and deliver personalised interventions in line with NICE guidelines. Support clients in achieving stability, independence, and improved mental health. Perform risk assessments, formulate risk management plans, and respond to acute episodes, including safeguarding concerns and mental health crises. Engage in home visits and urgent clinical interventions as required.
Work in partnership with psychiatrists, social workers, psychologists, and third-sector organisations to deliver integrated support.
Support medication adherence, administer depot injections, monitor side effects, and offer psychoeducation around pharmacological treatments.
Maintain accurate, timely records using the Trust’s electronic patient record system. Participate in clinical audits, service improvement meetings, and reflective supervision.
Registered Mental Health Nurse (RMN) with active NMC registration.
Proven experience in adult community mental health or recovery-based services. Strong clinical decision-making and risk assessment skills. Familiarity with Care Act, Mental Capacity Act, Mental Health Act, and safeguarding legislation. Capable of lone working in community and home settings. Previous experience as a Care Coordinator under CPA. Trained in CBT, DBT, or other structured therapeutic modalities. Access to own vehicle and full UK driving licence. Prior work in CMHT, CMHRS, or related community roles.
Person-centred, culturally competent, and recovery-focused. Resilient, flexible, and skilled in navigating complex emotional situations. Excellent verbal and written communication.
