We have a very exciting opportunity to recruit to a Band 5 Community Psychiatric Nurse role working within our Lincoln Community Mental Health Team.
We are looking for a motivated and enthusiastic RMN to join our outstanding multi-disciplinary community team which provides support for residents in the North of Lincoln within the Imp PCN.
Lincoln's Adult Community Mental Health Services are a committed integrated service that strive to provide high quality recovery-based support to people with severe and enduring mental health related difficulties.
The successful applicant will be providing high quality person-centred care to patients, families and carers. You will act as care co-ordinator, undertaking comprehensive evidence-based assessments and have a sound knowledge of Psycho-social interventions, supporting service users to work towards recovery goals, build self-resilience and promote independent living.
The post holder will be working within a friendly Multi-Disciplinary Team which includes Community Mental Health Nurses, Recovery Practitioners, Occupational Therapists, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists and Admin staff. You will also be working alongside Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries and other partner organisations to ensure the best outcomes for patients and carers.
The successful applicant will be offered robust clinical and managerial supervision on a regular basis in order that they may complete their role to the best of their ability and with sound knowledge and confidence. Additional training courses are available and you will be encouraged to participate to continue with your own Professional Development and to keep up-to-date with the most recent evidence-based practice.
