Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Physiotherapist to join our Proactive Frailty Care Planning Clinics. You will deliver Comprehensive Geriatric Assessments and autonomously assess therapy needs, within established protocols and MDT frameworks, to help patients lead healthier, more independent lives.

Experience in community services within Buckinghamshire or similar areas is advantageous, and the role offers NHS benefits, learning opportunities, and a supportive workforce

Qualifications

• Experience of delivering community services that help patients lead healthier, more independent lives.
• Autonomous practitioner working within frailty clinics.
• Proficiency in assessing frailty and identifying therapy needs.

Responsibilities

• Provide expert care and treatment to patients within agreed protocols and clinical guidelines.
• Undertake frailty health assessments and provide advice on therapy requirements.
• Work autonomously within own competence while part of an MDT.
