Qualifications

Responsibilities

• Assess referrals to the Frailty Hospital at Home service and prioritise by urgency.
• Develop comprehensive care plans with patients, carers, and the multi-disciplinary team.
• Perform risk assessments to ensure safe home-based care and timely discharges from urgent care settings.
• Provide evidence-based clinical/therapeutic interventions to improve health outcomes.
• Collaborate with community agencies and staff across Community Adult Health Services.
