Job summary

East Lancashire Hospitals NHS Trust is seeking a Band 6 Physiotherapist to join our community rehabilitation team. The role involves delivering rehabilitation in patients' homes across the East Lancs footprint, following discharge from acute care, and supporting long-term condition management.

You will work autonomously with an MDT, supervising staff, providing specialist assessment and treatment, and coordinating discharge planning.

Qualifications

• Band 6 level physiotherapist in community rehabilitation.

Responsibilities

• Assess, intervene and discharge a caseload of patients in their own homes.
• Supervise staff and provide support and expertise to MDT colleagues.
• See patients in their homes and local community, delivering specialist assessment and treatment.
• Prescribe essential equipment as part of therapeutic intervention and coordinate discharge planning.
• Maintain links with PCN and MDT teams for neighborhood-based care.
