East Lancashire Hospitals NHS Trust is seeking a Band 6 physiotherapist to join a supportive community rehabilitation service. You will assess, treat and discharge patients in their own homes, delivering rehabilitation across the community and supporting long-term condition management.

You will work autonomously with supervision of junior staff, collaborating closely with MDT and PCN teams, and participate in cross-locality working within the East Lancs footprint.

Qualifikationen
