Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Band 5 Community Therapy Physiotherapist to join the North Team in Fleetwood. You will support rehabilitation in community settings, helping clients regain independence and confidence through person-centred plans.

The role requires HCPC registration, physiotherapy qualification, and experience in community-based rehab. A flexible, patient-centred approach and a commitment to collaborative MDT working will be essential.

Qualifications

• Qualification in Physiotherapy
• Experience in community-based rehabilitation or recovery work
• Strong communication, empathy, and problem-solving skills
• Ability to motivate and encourage individuals at varying stages of recovery
• A commitment to holistic, strengths-based rehabilitation

Responsibilities

• Provide person-centred rehabilitation therapy in clients’ homes and community settings
• Support clients to develop daily living skills, mobility, confidence, and independence
• Deliver structured intervention plans and monitor progress
• Work collaboratively with physiotherapists, OTs, nurses and wider MDTs
• Facilitate group-based rehabilitation, wellbeing sessions, or community outreach and in-reach
• Promote safety, dignity, and empowerment in every aspect of care
