Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Band 5 Physiotherapist to join the Integrated Community Team. The role offers home-based and community rehabilitation with supervision and preceptorship support for those transitioning into community practice.

You will assess and treat adults, provide mobility and balance rehabilitation, and work with a multidisciplinary team to help patients maintain independence and quality of life.

Qualifications

• Degree or Diploma in Physiotherapy, state registered.
• Current UK professional registration.
• Experience in community/home-based rehabilitation is advantageous.

Responsibilities

• Assess, treat and support adults with a wide range of health needs.
• Provide assessment and rehabilitation interventions, including mobility assessment, strength and balance rehabilitation, exercise prescription, falls prevention, and functional recovery.
• Manage a caseload with supervision and work with health and social care professionals to support people to remain at home.
• Participate in CPD and provide supervision to rehabilitation assistants and students.
• Work with a multi-professional Locality Health Team to deliver community rehabilitation programmes.
