Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Community Physiotherapist to join a multidisciplinary team providing assessment, rehabilitation and support in the community. You will work with adults to maximise independence and quality of life, delivering programmes across mobility, strength and balance, and falls prevention.

The role includes supervision, preceptorship support, and a range of learning opportunities designed to develop practice in community settings.

Responsibilities

• Assess and plan comprehensive rehabilitation programmes in the community.
• Deliver mobility assessment, strength and balance rehabilitation, exercise prescription and falls prevention.
• Manage a caseload with supervision and work with senior clinicians on complex cases.
• Provide CPD, supervise rehabilitation assistants and students on placement.
• Collaborate with partners in health and social care to support patients at home and prevent deterioration.
