Job summary

Sanctuary Personnel are seeking an Occupational Therapist to join our Community Team in Oxford. This is a full-time, ongoing role requiring HCPC registration and a driving licence, with a salary of £38.00 hourly.

Flexible scheduling is available to help you balance work and life. You will assess individuals in their homes after hospital discharge, facilitate timely discharges, perform functional assessments, and recommend adaptive equipment to improve independence.

Qualifications

• HCPC registration is required.
• Driving licence required.
• Experience in community settings is preferred.

Responsibilities

• Assess individuals in their homes following hospital discharge.
• Facilitate safe and timely discharges from hospital through the Discharge to Assess pathway.
• Complete functional assessments and conduct in-depth activity analyses.
• Recommend and prescribe adaptive equipment, assistive technologies, and minor modifications to improve independence.
• Perform moving and handling assessments and conduct regular reviews to ensure safety and care quality.
• Collaborate with multidisciplinary partners to develop and implement personalised support plans.
• Encourage independence and reduce dependency on ongoing care where feasible.
• Assist individuals, families and carers during transition and recovery periods.
• Maintain comprehensive and timely case records to ensure high standards of care.
