Job summary

Sanctuary Personnel is seeking an Occupational Therapist to join the Community Social Care Team in Oxford on a locum, full-time ongoing basis. The role offers £38 per hour and involves home-based assessments, discharge planning, and collaborative care planning to promote independence and safe hospital discharges.

You will conduct functional assessments, prescribe equipment or minor adaptations, and coordinate with MDT partners while maintaining thorough records.

Qualifications

• Must be HCPC registered with current registration.
• Driver's license required for community travel.
• Experience in community/home-based settings is preferred.

Responsibilities

• Assess individuals in their own homes following hospital discharge.
• Support safe and timely hospital discharges through the Discharge to Assess pathway.
• Conduct comprehensive functional assessments and activity analysis.
• Recommend and prescribe equipment, assistive technology, and minor adaptations.
• Undertake and review moving and handling assessments.
• Collaborate with multidisciplinary partners to develop personalized support plans.
• Promote independence while minimising ongoing care when appropriate.
• Assist individuals, families and carers during transitions and recovery.
• Maintain accurate and timely case records.
