Job summary

Sanctuary Personnel is seeking an Occupational Therapist to join a Community Social Care Team in Newham, London. This full-time ongoing locum role offers hybrid working with community-based assessments up to three days per week, at a rate of up to £38.00 per hour.

Responsibilities

• Complete Adult Social Care Occupational Therapy assessments to ensure personalised care plans.
• Undertake DHC to SHC reviews and optimise packages of care for individuals in need.
• Carry out complex moving and handling assessments with a focus on safety and effectiveness.
• Collaborate closely with home care providers, care coordinators, individuals, and families.
• Assess for and recommend the use of specialist equipment to enhance daily living.
• Conduct postural and seating assessments for improved comfort and functionality.
• Design and produce basic drawings for standard adaptations including LAS.
• Evaluate and present recommendations for major adaptations where necessary.
• Manage an independent caseload while meeting service deadlines.
• Prepare comprehensive reports, maintain accurate records, and handle correspondence professionally.
• Liaise with service users, carers, and multidisciplinary professionals for cohesive care delivery.
• Support Occupational Therapy students and support workers, providing guidance when appropriate.
