Qualifications

• Degree or Diploma in Occupational Therapy recognised by the HCPC.
• Evidence of continuing professional development relevant to Adult Social Care practice.
• Current registration with the Health and Care Professions Council (HCPC).
• Experience as an Occupational Therapist within Adult Social Care, community services, rehab, or health and social care settings.
• Experience undertaking complex assessments and developing person-centred support plans.

Responsibilities

• Undertake comprehensive occupational therapy assessments to identify needs, risks and opportunities to maximise independence.
• Recommend and arrange equipment, adaptations, assistive technology, and moving and handling solutions.
• Complete assessments and reviews in accordance with the Care Act 2014 and related Adult Social Care legislation.
• Collaborate with social workers, NHS professionals, housing providers, carers and community organisations to coordinate support plans.
