Job summary

Pennine Care NHS Foundation Trust in Greater Manchester is seeking an Occupational Therapist to join the Community Rehabilitation Team on a temporary contract. The post covers the five borough footprint and focuses on MOHO-informed assessment and intervention within a multidisciplinary team.

You will manage a caseload, deliver OT assessments and group/individual interventions, contribute to discharge planning, and supervise junior staff.

Qualifications

• Must hold a diploma or degree in Occupational Therapy and be registered with HCPC.
• Evidence of post qualification experience and continuing professional development to post graduate diploma level.
• Willingness to undertake further training relevant to the role resulting from legislative or service changes, as appropriate.

Responsibilities

• Embed the OT assessment and intervention pathway across the Community Rehabilitation Team, within the MOHO framework.
• Manage a caseload of service users and support their recovery journey.
• Undertake specialist OT assessments in self-care, productivity and leisure for discharge planning.
• Develop outcome-based, needs-led care plans and evaluate effectiveness using MOHO tools.
• Provide a range of group and individual therapeutic interventions and share skills with the team.
• Develop treatment plans for delivery by social inclusion workers and provide supervision to junior staff.
