Responsibilities
- Undertake strength-based reviews and assessments in line with the Care Act.
- Collaborate with service users, carers, and multi-disciplinary teams.
- Recommend and coordinate equipment provision.
- Contribute to service development in the Review Team.
- Maintain accurate records and ensure compliance.
