Job summary

Qualifications

• HCPC registration as an Occupational Therapist.
• Experience working within community/social care settings.
• Strong assessment and clinical reasoning skills.
• Ability to manage a caseload effectively.
• Excellent communication and MDT working skills.
• Free DBS and compliance service.

Responsibilities

• Complete Occupational Therapy assessments for adults with a range of needs.
• Assess functional ability and identify barriers to independence.
• Develop person-centred intervention plans.
• Recommend equipment and adaptations where required.
• Complete risk assessments and support safe decision-making.
• Promote independence and wellbeing through strengths-based practice.
• Work collaboratively with multidisciplinary teams and partner agencies.
• Support service users and carers to achieve agreed outcomes.
• Maintain accurate and timely case records.
