Job description

We are seeking an experienced and motivated Community Nursing Team Lead to play a key role in the success of our Community Nursing Teams. This leadership post is for an NMC-registered nurse with a Specialist Practitioner Qualification (SPQ) who demonstrates the Trust’s values and behaviours.

You will lead, develop and operationally manage a designated Community Nursing Team, ensuring the delivery of high-quality, evidence-based, person-centred care across a seven-day service. This includes managing capacity, prioritising visits and responding effectively to service pressures.

The role involves supervising, supporting and developing staff, promoting interdisciplinary working, professional development and lifelong learning. Working closely with the Clinical Lead and stakeholders, you will support service integration, maintain CQC standards, manage delegated budgets, oversee recruitment and retention, and ensure compliance with policies, performance targets and contractual requirements.

You will demonstrate strong communication skills, sound clinical judgement and the ability to manage risk, investigate incidents and complaints, and embed learning across the service.

You will maintain your own clinical competencies, NMC registration and professional portfolio while providing visible clinical leadership to teams managing complex caseloads.

  • Provide clinical and operational leadership for a Community Nursing Team, ensuringhigh standardsof safe, person-centred care.
  • Manage day-to-day service delivery, including capacity planning, prioritisation of visits, seven-day service cover, and escalation of risks or pressures.
  • Lead, supervise and develop staff through appraisal, performance management, clinical supervision,educationand training.
  • Recruit,retainand support staff wellbeing, attendance, and professional development.
  • Ensure compliance with CQC, organisational policies, clinical governance, safeguarding, health and safety, and risk management requirements.
  • Manage delegated budgets, resources and staffing efficiently.
  • Maintain effective communication within the team and with internal and external stakeholders, positivelyrepresentingthe service.
  • Investigate complaints,incidentsand serious incidents,identifyingroot causes, implementingimprovementsand sharing learning.
  • Contribute to service improvement, quality assurance, audits, datareportingand strategic development of community nursing services.
  • Maintain professional registration, clinicalcredibilityand ongoing personal and professional development in line with NMC standards.
  • Provide clinical leadership, support and guidance for complex caseloads,triageand evidence-based community nursing practice.
