Job summary

Oxleas NHS Foundation Trust is seeking a Nursing Associate to join our Adult Community Physical Health Services in Leigh. The post is Band 4, permanent, full-time, 37.5 hours per week, with an included weekend rotation (one in four).

You will deliver high-quality, evidence-based care under supervision of registered nurses and work with multidisciplinary teams to support patients in community and home settings.

Qualifications

• Registered Nursing Associate on the NMC register.
• English and Mathematics basic qualifications (eg GCSE at Grade C or above or evidence of Level 2 in Functional skills for numeracy and literacy)
• Community Experience

Responsibilities

• Manage priorities as delegated by the registered nurse.
• Deliver individual care plans meeting immediate and ongoing needs.
• Participate in the delivery of complex care as prescribed by the registered healthcare professional.
• Plan and document the patients’ progress on the Trust’s information systems.
• Maintain professional records and communicate with multi-disciplinary teams.
• Teach and demonstrate equipment use and self-care to patients.
• Identify opportunities to improve service delivery.
