Job summary

Mid Cheshire Hospital Foundation Trust is recruiting for a Band 4 Assistant Practitioner/Nursing Associate to work within a CCICP community team. The role involves following care plans and delivering a range of clinical skills under supervision, including home visits and care in various settings.

You will support patients and informal carers, help reduce hospital admissions, and participate in training and governance activities to maintain high standards of care.

Qualifications

• Qualified AP/NA with foundation degree or equivalent.
• Ability to work under the supervision of Registered Nurses/Practitioners.
• Strong communication skills with patients and carers.

Responsibilities

• Deliver care as delegated within the CCICP team scope of practice.
• Support patients in their own homes, clinics or other settings.
• Promote independence and wellbeing and support carers and families.
